Our renovated facility offers a very comfortable and homelike environment for our residents and families. We focus on outcomes for our residents, and we measure trends and results so we can innovate and constantly improve the care we give.
We also offer:
Long-term skilled nursing
Short-term rehabilitation
Occupational therapy
Physical therapy
Speech therapy
IV antibiotic management
All of these services are available to residents in our nursing home at no extra charge. We accept both private pay and Medicaid payments for skilled nursing services.

We offer a 43-room, renovated and modern nursing home facility close to the local hospital, restaurants, shopping, churches, and an active senior center. Three Rivers Care has three private rooms and a total of 83 licensed beds. Our life enrichment activities offer a wide scope of choice and variety.

At Three Rivers Care, we offer high-quality skilled nursing and rehabilitation services designed to promote optimal recovery, independence, and overall well-being.
In our short-term rehab program, our therapists design a personal plan that focuses on restoring mobility, strength, and daily living skills for a swift return to normal activities.
Our experienced staff provides 24-hour senior care and skilled nursing in a newly renovated and comfortable nursing home. We offer ongoing, comprehensive attention for residents and our staff is constantly receiving input and continuing education so they can provide excellent senior care.
Our comprehensive therapy services lead the way to recovery after a life-altering event. We offer patients physical therapy, occupational therapy, wound therapy, and speech therapy services with a collaborative approach that has nurses, therapists, and physicians working together. We provide personalized care to support individuals in regaining their health and independence.
At Three Rivers Care, we take pride in our life enrichment services that include a therapy gym, lounge areas, free WiFi, free cable TV, a beauty salon, and many other amenities that make staying here enjoyable.
